Community Care and other Financial Assistance
Community Memorial Hospital can help you with your financial concerns. Our Patient Financial Services staff can guide you through the Community Care Program Application process and answer your questions about other financial assistance options that might be available to you.
Community Memorial Hospital's Community Care Program offers help in paying for services provided by the hospital to people who are experiencing financial difficulty and who meet eligibility requirements. Patients with balances as a result of limited or no insurance coverage may qualify for our Community Care program or other financial assistance programs.

CMH uses the federal poverty income guidelines issued by the U.S. Department of Health and Human Services to determine a person’s eligibility for Community Care. To determine if you are qualified for our Community Care program or other financial assistance programs, please contact one of our Patient Financial Services representatives at (920) 846-2947.
Insurance and HMO/PPO Information
Community Memorial Hospital makes every effort to participate in all heath insurance plans in our service area. If you have questions as to whether or not we participate in your plan, please call your insurance company or check your plan’s provider directory. It is very important to know if your insurance plan requires prior authorization for healthcare services you receive at CMH. Many insurance plans require the physician or policy holder to initiate the prior authorization process. Failure to obtain prior authorization may result in partial or complete denial of insurance payment making the balance your responsibility.
Worker's Compensation
Community Memorial Hospital bills your employer or the Worker's Compensation carrier that you provide at the time of registration. You must inform your employer of any Worker's Compensation claim for it to be covered. If a denial from your employer or Worker's Compensation carrier is received, the bill becomes your responsibility.
If You Do Not Have Insurance
We will ask you for a partial deposit at the time of service during the registration. The remaining balance will be billed to you shortly after you have received your healthcare services. If you are unable to make your payment in full, please contact our Billing Office at (920) 846-2521 to make payment arrangements.
Discounts for Uninsured
If you do not have insurance and pay your balance in full within 30 days of the date on your first statement, you are eligible to deduct 25 percent from the total amount due. For questions on our discount policy, please call the Billing Office at (920) 846-2521.
