Diabetes Education Program

The Diabetes Education Program at Community Memorial Hospital is designed to provide you with information and skills you need to know in order to manage your diabetes and take charge of your life. The first step in taking charge is learning about your diabetes.

Managing your diabetes is a team effort; you are not alone in your efforts. The health care professionals at CMH want to be a part of your team.

Our Diabetic Educator will show you how to feel better and improve the quality of your life.

It is not the end of the world to have diabetes. You can control this disease through meal planning, activity, medication, self-monitoring and education. It is not easy having diabetes, but once you learn how to manage it you can live an active, enjoyable life.

The CMH health care professional team will provide you with tools and information:

  • Understanding diabetes
  • Medication management including insulin management
  • Meal planning with carbohydrate counting
  • Exercise program
  • Glucose monitoring
  • Self blood glucose monitoring

Diabetes Support Group

A support group for individuals and families who are living with diabetes. Led by a diabetic educator, monthly meetings feature speakers, food sampling, and discussion.

  • Free and open to the public
  • Fourth Thursday of each month
  • CMH Community Room
